  • I  affirm my support of the SOGICE Survivor Statement, including its recommendations developed by survivors of SOGICE and the LGBTQA+ Conversion movement
  • I  consent to the publication of my name in the public list of signatories on the website. (This does not indicate that your name will necessarily be published).

If you are affirming on behalf of an organisation/group:

  • I/We  affirm our organisation/group support of the SOGICE Survivor Statement, including its recommendations developed by survivors of SOGICE and the LGBTQA+ Conversion movement.
  • I/We have the authority to submit this form on behalf of my/our organisation/group.
  • I/We consent to the publication of the name of my/our organisation in the public list of signatories on the website. This may include a logo or similar visual representation. (This does not indicate that your organisation/group name or logo will necessarily be published).
